Job Description
A Care Coordinator at Sword Health is responsible for supporting patients through their musculoskeletal pain treatment plans, coordinating care, and ensuring a positive patient experience within a fast-paced, innovative digital healthcare environment.
Key Responsibilities
- Coordinate care plans for patients with musculoskeletal pain
- Collaborate with licensed physical therapists and digital therapists to support patient progress
- Monitor patient progress and adjust care strategies as needed
Requirements
